A representative from the home-based healthcare agency schedules an assessment and consultation with the individual and/or their caregivers. Care Plan Development
Based on the assessment findings and individual preferences, the healthcare professional collaborates with the individual and their caregivers to develop a personalised care plan. Service Agreement and Authorisation
Once the care plan is finalised, the individual or their authorised representative reviews and signs a service agreement outlining the terms, conditions, and expectations of home-based healthcare services. Coordination of Services
